This Privacy Statement outlines our practices regarding the collection, use and disclosure of personal information.
What does the term "personal information" mean?
"Personal information" means information about an identifiable individual, but does not include the name, title or business address or telephone number of an employee of an organization.
"Personal Information" does include personal health information such as:
- information concerning physical or mental health;
- information concerning any health service provided;
- information concerning the donation of any body part or bodily substance;
- information derived from the testing or examination of a body part or bodily substance;
- information collected in the course of providing health services;
- information collected incidentally to the provision of health services.
We gather personal information including:
- Contact information (e.g. name, address, e-mail address, telephone number);
- Demographic information (e.g. gender, date of birth, occupation);
- Proof of identity (e.g. signature; photo ID);
- Medical information such as medical history, medical examination
results and diagnostic testing) from:
- Referring Sources;
- Examinees;
- Third party service providers;
We use personal information in connection with our services:
- To confirm identity
- To conduct evaluations of current and prior conditions that may impact on illness or injury
- To communicate with an examinee
We do not use personal information about an Examinee for any other purposes without seeking the Examinee's prior consent.
Sharing of personal information:
MedAccess Solutions may share personal information with third parties engaged to assist it in carrying out the IME services. For example, a limited amount of personal information (name, contact information and language preferences) will be shared with a translator assisting in examination of a non-English speaking examinee. Personal information (including personal health information) is shared with medical services providers who perform medical, ergonomic and mental health evaluations. These providers are prohibited from using personal information for any purpose other than in provision of this service, and are required to protect personal information disclosed by MedAccess Solutions in accordance with the privacy principles described in this Privacy Statement and in accordance with applicable laws.
Personal information about an examinee, including personal health information obtained through medical examinations and additional diagnostic testing, is shared with the examinee's referring source. This is always done with the consent of the examinee.
Disclosure of personal information:
MedAccess Solutions does not disclose personal information for purposes unrelated to our services. MedAccess Solutions reserves the right to disclose personal information to a third party if a law, regulation, search warrant, subpoena or court order legally requires or authorizes it to do so.
Knowledge and consent:
MedAccess Solutions collects personal information only with the consent of the examinee. An examinee may withdraw consent at any time provided there is reasonable notice. This is subject to any legal or contractual restrictions. The examinee may do so by contact MedAccess Solutions as shown below. As much as possible, the examinee will be informed of the implications of withdrawing consent.
We protect the examinee's personal information:
Access to personal information, whether hard copy or electronically stored, is restricted to MedAccess Solutions personnel and an examinee's representatives on a strict need-to-know basis. We deploy standard information security techniques to guard personal information from loss and unauthorized access.
Retention of Personal Information:
Personal information is retained by us as required to fulfill the purposes for which it was gathered and to comply with applicable laws.
Changes to this Privacy Statement:
We reserve the right to modify this Privacy Statement at any time, without notice. At any given time the Privacy Statement distributed shall be deemed to be the Privacy Statement then in effect.
